1. Acknowledgment of Risk:
I acknowledge that participating in tennis coaching and related activities involves inherent risks, including but not limited to injury, illness, and even death. I understand these risks may result from my actions, the actions of others, or the condition of the facilities or equipment.
2. Waiver and Release:
In consideration of being allowed to participate in tennis coaching, I hereby release, waive, and discharge Frantz Glasz dba CAL Tennis, its coaches, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property, whether caused by the negligence of the released parties or otherwise, while participating in tennis coaching.
3. Indemnification:
I agree to indemnify and hold harmless Frantz glasz dba CAL Tennis from any loss, liability, damage, or costs they may incur due to my participation in tennis coaching, whether caused by negligence or otherwise.
4. Medical Attention:
I consent to receive medical treatment in the event of injury, accident, or illness during my participation in tennis coaching. I understand that I am responsible for my own medical expenses.
5. Photo and Video Release:
I grant permission to Frantz glasz dba CAL Tennis to take photographs and videos of me during coaching sessions. I understand that these images may be used for promotional purposes, including but not limited to websites, social media, and marketing materials. I waive any right to inspect or approve the finished images or their use.
6. Governing Law:
This Waiver shall be governed by and construed in accordance with the laws of the state of California.
7. Severability:
If any provision of this Waiver is found to be unenforceable or invalid, the remaining provisions shall continue in full force and effect.
I HAVE READ THIS WAIVER OF LIABILITY AND PHOTO RELEASE FORM AND UNDERSTAND ITS TERMS. I SIGN IT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
